  • Are you passionate about helping people successfully reintegrate into the community after prison?

    The Forward Trust is looking for a dedicated RECONNECT Worker to provide a bespoke, inclusive service to clients across HMP Bedford, HMP Woodhill, and Yarl’s Wood IRC, ensuring they have the guidance and support needed to thrive post-release.

    About the Role:

    As a RECONNECT Worker, you’ll work directly with service users in these locations to identify their needs, create personalised care plans, and support them for up to six months after release. You’ll coordinate with multiple agencies to provide interventions across physical and mental health, wellbeing, mentoring, GP referrals, aftercare, and connections to the wider Forward Trust community.

    Key Responsibilities:

    • Process referrals and provide updates to referrers on assessment outcomes.
    • Conduct in-depth needs assessments for all suitable referrals.
    • Maintain contact and support for service users in preparation for release and for six months post-release.
    • Build and maintain excellent partnerships with external agencies and key stakeholders.
    • Implement clear referral pathways, including Offender Management in Custody, Family/Self, Prison, and Healthcare.
    • Deliver a holistic service safely in line with individual and team targets.
    • Collaborate with colleagues to share best practice and fully engage in risk assessment processes.
    • Ensure service users are fully connected with local services before release into the community.

    What We’re Looking For:

    • Experience in working with offenders, vulnerable adults, or in a similar support role.
    • Strong communication and relationship-building skills.
    • Ability to deliver person-centred, solution-focused support.
    • Knowledge of relevant statutory and voluntary services.
    • Commitment to safeguarding and promoting wellbeing.
