The Forward Trust is seeking a compassionate and motivated Health and Wellbeing Practitioner to join our integrated healthcare team at Heathrow IRC on a part-time basis (21 hours per week). You'll support detainees with substance misuse, low-level mental health needs and wider wellbeing concerns, helping them build resilience and prepare for the next stage of their journey.
What you'll be doing
Working as part of a multi-disciplinary team, you will:
- Complete assessments, risk management plans and recovery plans
- Manage a caseload and deliver both 1:1 and group interventions
- Support individuals with substance misuse and mental health needs
- Develop release, discharge and continuity of care plans
- Promote wellbeing services and signpost to relevant support
- Contribute to multi-disciplinary reviews and daily team meetings
- Maintain accurate case records and reports
- Provide transitional support to individuals preparing for release or deportation
