About the role
We are looking for a compassionate, resilient and proactive Outreach Worker to engage with people who are rough sleeping or begging, including those who may be reluctant to engage with services or have multiple and complex needs.
You will provide assertive and flexible street outreach, building trusting relationships and supporting individuals to access accommodation, healthcare, mental health and substance misuse services. You will maintain accurate records, contribute to multi-agency meetings and case planning, and ensure individuals receive coordinated support tailored to their needs.
