We’re looking for a passionate and proactive individual to join The Forward Trust as a RECONNECT Worker, supporting individuals preparing to leave HMP Woodhill, HMP Bedford, and IRC Yarl’s Wood.
RECONNECT is about building bridges and ensuring people have the support they need to safely transition back into the community.
You’ll provide tailored, person-centred support to individuals in custody and for up to 6 months post-release, linking them into vital services including healthcare, mental health support, housing, and wellbeing interventions.
You’ll be responsible for assessing needs, creating bespoke care plans, making referrals, and staying in regular contact to ensure each person is connected to the help they need. You’ll work closely with prison staff, healthcare providers, local agencies and our wider Forward community.
