- Support clear referral pathways across all settings
- Contribute to monitoring, reporting, KPIs and quality standards
- Work collaboratively with healthcare, probation and voluntary sector partners

- Assist with safeguarding processes and risk monitoring
What We’re Looking For
- Strong administrative and data management skills
- Experience using secure databases and case management systems
- Excellent organisation and attention to detail
- Ability to prioritise and meet deadlines
- A trauma-informed, non-judgemental communication style
- A collaborative team player committed to high-quality service delivery
By joining RECONNECT, you’ll become part of a purpose-driven service that plays a vital role in breaking the cycle of crime. Your work will directly support safer transitions from prison to community healthcare, helping individuals access coordinated, person-centred support at a critical time in their lives. You’ll work within a collaborative, values-led team committed to quality, continuous improvement and making a genuine, measurable difference every day.
