    About the Role

    As a Health and Wellbeing Practitioner, you'll manage your own caseload and deliver high-quality psychosocial interventions to adults experiencing substance misuse issues.

    Working within a multidisciplinary team, you'll complete assessments, develop SMART care plans and deliver structured one-to-one and group interventions, using both harm reduction and recovery-focused approaches tailored to individual needs.

    You'll work closely with healthcare professionals and partner agencies to ensure clients receive holistic support that addresses wider issues including housing, employment, benefits and family relationships.

    Flexibility is essential within this role. This may include occasional evening or weekend working and, with reasonable notice, travelling to other services within your cluster to support service delivery and ensure continuity of care.

    What You'll Be Doing

    • Managing a caseload of clients throughout their treatment journey.
    • Completing comprehensive assessments, risk assessments and care planning.
    • Delivering structured one-to-one interventions and facilitating group programmes.
    • Providing harm reduction advice, guidance and relapse prevention support.
    • Motivating and supporting clients to engage with treatment and recovery services.
    • Working collaboratively with healthcare professionals and external agencies to provide holistic support.
    • Referring clients to services including housing, employment, education, healthcare and family support.
    • Monitoring client progress and maintaining accurate case records.
    • Contributing to service performance targets and positive client outcomes.

    About You

    We're looking for someone who is passionate about supporting people to make positive changes in their lives.

    You'll have experience working within substance misuse services and be confident delivering structured interventions that place the individual at the centre of their recovery.

    You'll Have:

    • Experience working within drug and alcohol or substance misuse services.
    • Experience completing comprehensive assessments, risk assessments and developing SMART care plans.
    • Experience delivering structured one-to-one interventions.
    • Experience facilitating therapeutic groups.
    • Experience using motivational interviewing techniques in both one-to-one and group settings.
    • Strong IT and case recording skills.
    • An understanding of holistic care, continuity of care and partnership working.
    • A commitment to equality, diversity and inclusion.

    Knowledge & Qualifications

    Essential knowledge includes:

    • Understanding of the challenges faced by people affected by substance misuse.
    • Knowledge of recovery-focused practice.
    • Knowledge of safeguarding principles and procedures.

    Essential qualification:

    • Level 3 Diploma in Health and Social Care (or equivalent).
