The Forward Trust delivers trauma-informed, gender-responsive Health and Wellbeing services across Surrey prisons in partnership with NHS healthcare providers, and we’re looking for a dedicated Health & Wellbeing Practitioner to join our team at HMP Downview, a closed adult female prison.
What you’ll do:
- Manage a caseload, providing tailored assessments, care plans, and recovery-oriented interventions.
- Deliver structured 1:1 sessions and group-work, supporting clients with substance misuse and low-level mental health needs.
- Provide harm reduction guidance and second signatory duties.
- Support release planning and transitional care for service users returning to the community.
- Work within a multidisciplinary team, collaborating with mental health, healthcare, and prison staff.
- Contribute to KPIs, audits, and the continuous improvement of our Health & Wellbeing services.
What we’re looking for:
- Experience in substance misuse, mental health, or related health and social care fields.
- Strong interpersonal and communication skills, with the ability to work collaboratively.
- Flexibility to work on a rota including weekends.
- Ability to manage your caseload effectively and maintain accurate records.
- Commitment to trauma-informed, gender-responsive, and recovery-oriented approaches.
